For Professionals: Refer a Patient

Safety. Privacy. Healing. A second chance at life.

You want what’s best for your patients. As a behavioral healthcare professional, you understand that effective treatment is rarely one-size-fits-all. Each patient’s journey is unique, often requiring a nuanced, multidisciplinary approach that evolves over time.

At Discovery Addiction Centers, we recognize the complexity of substance use disorders and co-occurring conditions, which is why we offer a robust continuum of care designed to meet patients exactly where they are clinically, emotionally and psychologically.

Measurement-Based Care: At the Heart of What We Do

Our system provides seamless access to varying levels of care, including:

At every level, your patients will benefit from our unique measurement-based care approach. We carefully measure 27 distinct wellness indicators, including mental and physical health histories, social relationships, history of substance use and more.

This allows us to precisely and effectively meet your patients’ needs as they progress through our programs, giving them the best chance they need at a full recovery and a true second chance at life.

And then there’s Discovery365™. Our revolutionary recovery platform uses groundbreaking AI technology to help prevent relapse from happening for a year after they leave our care. It connects them to resources and even lets them know when they need to reach out for support.

When Should I Refer My Patient?

There are key indicators that may signal when it’s time to refer a patient to a higher level of care. These include:

Stalled Progress

The patient has reached a therapeutic plateau and is no longer making meaningful strides toward their goals.

Resistance to Growth

They may be actively resisting change, creating barriers to treatment because they feel safe in their current setting and are reluctant to move forward.

Environmental Disruptions

External factors (such as family dynamics, peer pressure, academic or workplace stress, or a generally unsupportive environment) are interfering with their ability to follow through on therapeutic directives.

If you observe signs of regression or feel that your patient’s current level of care is no longer sufficient, a referral to a more intensive program may be appropriate. At Discovery Addiction Centers, we’re equipped to provide that next step. Our team ensures a smooth and respectful transition into our care, and we’ll keep you informed every step of the way.

When your patient is ready for discharge, we’ll coordinate a thoughtful handoff back to your practice, ensuring continuity and support through aftercare, because healing doesn’t end when treatment does.

You remain a vital part of their recovery journey, and we’re here to help reinforce that connection.

Partner with Us to Help Patients Thrive

Our goal is not only to stabilize and support patients during their time with us, but also to prepare them for successful reintegration into your care once they complete their program. We view our role as a collaborative partner: one that complements your work and helps patients build the resilience and skills needed for long-term recovery.

Together, we can create a treatment ecosystem that prioritizes continuity, compassion and clinical excellence: empowering patients to reclaim their lives with confidence and support.
